Financial Policy:

We strive to keep our office fees as reasonable as possible for our clients.  Payment is to be received the day that services are rendered for patients without insurance coverage.  Payment plans are available for larger treatment plans.  This can be arranged through our receptionist as needed.

Insurance Information:

For billing purposes, we will need your assistance to make this process as smooth as possible. We ask that you do the following:

• Bring your Insurance ID card(s)
• Sign the Signature on File form(s)
• Sign the HIPAA form

Your insurance is an agreement between you and your insurance company.  We are pleased to process your insurance claims with the understanding that you, the patient, are ultimately responsible for any expenses incurred.
